Application for Use of Thrivent Choice Dollars Funds

Funds will be disbursed on the following dates:

March 15 Application must be received by March 1
June 15 Application must be received by June 1
September 15 Application must be received by September 1
December 15 Application must be received by December 1

Brief Instructions for filling out application.

Contact Person - Please list the person who is leading the project or event. Multiple contact persons may be listed, if applicable.

Ministry Sponsoring Event/Project/Activity - all projects must be sponsored by a ministry team of the congregation, as per the distribution policy. If it is unclear which ministry team would sponsor a given project, groups can request their project be sponsored through the church council.

Estimated Costs - Please note all costs associated with the project or event. If requesting "seed money" for a new ministry or project, please indicate both the inital start-up cost as well as any ongoing costs that may be required to keep the ministry going once the Choice Dollars funds are exhausted. If the event is a fund raiser, please indicate estimated costs of putting on the fund raiser (e.g. for Soup Suppers, cost of ingredients, condiments, etc.)

Total Funds Requested - List the total amount of Thrivent Choice Dollars your group is requesting
